02 · Four testing pathways

Blood cancers · Blood 600
Twin Blood 600. A molecular pathway for blood cancers.
Overview
What this pathway is
OnKommon Twin · Twin Blood
DNA and RNA together, from blood or bone marrow.
Twin Blood 600 is the Twin testing route for blood cancers. It analyses DNA and RNA together, from a blood or bone-marrow sample. Reading both in one route matters in blood cancers, where mutations and gene fusions both drive the disease.
Intended audience and use
Who it is for
People with a blood cancer whose treating haematologist or oncologist is considering molecular profiling.
Sample and testing pathway
Sample and what is generated
- Sample
- Blood or bone marrow.
- Information generated
- DNA and RNA findings — mutations, fusions and expression — with the biomarkers integrated into the Twin.
From result to Twin
How the information enters Twin
Findings become the molecular foundation of the baseline Twin (TwinBirth) and are read through the analytical layers relevant to the data — for example SourceGuard for origin, RNA Expression & Signatures for fusion and expression information, and AccessMatch and TrialGraph for evidence and trials.
Analytical layers most relevant to this route
- 2SourceGuardSomatic and germline interpretation
- 1CloneTraceClonality and tumour evolution
- 10RNA Expression & SignaturesExpression patterns and signatures
- 6AccessMatchActionable alterations and targetability
- 13Staging & Prognostic ModelsStage, risk models and outcome estimates
- 16TrialGraphClinical trial matching and prioritisation
- 15TwinBirthCreation of the patient's digital twin
Strengths
Why this route
- DNA and RNA in one route — mutations and fusions read together.
- Blood or bone marrow — no solid-tissue biopsy needed.
- Built for haematological cancers, where fusions often define the disease.

Deliverables
What you receive
- A structured Twin report in the five documented components: executive molecular summary, molecular findings, treatment options to evaluate, clinical-trial matches and a longitudinal view.
- Review and sign-off by the treating oncologist, within the KPCIRC sign-off process.
- Entry into the six-month Twin journey, with the option to continue into a Twin track afterwards.
The treating oncologist owns every decision. Every Twin report is reviewed and signed by the treating oncologist, then taken to KPCIRC's Molecular Tumour Board.
