Longitudinal intelligence

The cancer journey evolves. The evidence evolves with it.

Twin is longitudinal, not a one-time genomic report. It establishes a baseline, keeps findings under review for six months, and then sets the next pathway.

Baseline — Day 0

Integrate the data. Build the Twin.

The testing-pathway result and the information supplied by the treating team are integrated, and the baseline Twin is established. What is missing is recorded.

Lifecycle

Nine stages, from entry to updated view

What happens at each stage, and where review and ownership sit.
  1. 1

    Entry into the appropriate pathway

The treating oncologist selects Blood 600, Exome WES, Liquid 523 or Tissue 335 for the clinical question.

  2. 2

    Data collection and quality review

The sample is tested and the available clinical, molecular and imaging information is gathered. Every input passes a data-quality review before it enters the Twin.

  3. 3

    Molecular and clinical information processing

Results and clinical context are integrated to establish the baseline Twin at Day 0.

  4. 4

    Analysis through applicable layers

AI-assisted analysis runs across the layers relevant to the available data.

  5. 5

    Structured interpretation

Findings are organised into the five report components, with evidence and uncertainty.

  6. 6

    Required clinician review

The treating oncologist reviews and signs the report; it is then taken to KPCIRC's internal Molecular Tumour Board.

  7. 7

    Initial outputs

The signed report becomes the reference point for what follows.

  8. 8

    Follow-up and new information

During months 0–6, new findings are reviewed inside the workflow; new evidence can prompt reassessment.

  9. 9

    Reassessment and updated outputs

Where new data or evidence warrant it, the Twin is updated and versioned. At the end of month six, the evidence is reviewed and the next pathway set.

How the Twin stays alive

Every result in. A genetic read every three months.

Longitudinal monitoring brings every change into the Twin as it happens — platelets, LFT and KFT panels, IHC reports, scans, symptoms and treatment changes, shared by the patient or the care team by upload or on WhatsApp.

Track monitoring adds OnKommon's genetic tracking tests every three months — each with its own designed panel, ranges and integrated biomarkers. Together, the two streams keep the Twin current for as long as the package runs.

After six months

Twin tracks

Track 12, Track 47 or Track 118. Genetic tracking tests performed by OnKommon — each with its own designed panel, ranges and integrated biomarkers — run every three months.

The treating oncologist owns every decision. Every Twin report is reviewed and signed by the treating oncologist, then taken to KPCIRC's Molecular Tumour Board.

Patient journeys

Three ways a Twin lives.

Examples of packages a treating oncologist can design — scroll to follow one patient through twelve months.

  1. M00Exome (WES) sampleThe tumour is sequenced across every protein-coding gene. The Twin is created at Day 0.
  2. M01PlateletsA new blood count arrives on WhatsApp and is added to the Twin.
  3. M02LFT panelLiver function results are uploaded and read against the treatment history.
  4. M03IHC panelA new IHC report joins the molecular picture.
  5. M04CT scanImaging is added and change over time is mapped.
  6. M06Month 6 · Twin + Track 118A Track 118 genetic read and a full Twin review. The oncologist sets the next step.
  7. M08New symptomA new symptom is mapped onto the Twin and connected to the evidence.
  8. M10LFT panelFollow-up results keep flowing in.
  9. M12Month 12 · Twin + Track 118A second Track 118 read and a full Twin review — a year of the cancer journey, in one model.
